- You will receive a symptom questionnaire upon checking out. Please fill out both sides completely and email it back to us at support@innovatewellness.com. We will process it and email you your lab test form within 7 business days with all directions needed to get blood drawn (no additional payment).
- It will take approximately 2 weeks to get your labs back and receive your custom supplement protocol and lab analysis recording.
- You will also get instructions about how you can order your supplement protocol.
